Our Office Policies
At The Allergy Center at Brookstone, we promise to provide you with the best possible healthcare available, and to make your experience in our office as pleasant as possible.


Office Hours

Columbus Office: 

Monday - Friday:     8:00 a.m. - 5:00 p.m.
Closed for Lunch:   12:30 p.m. - 1:30 p.m.

"Injection Only" hours differ - See Injection Schedule

Lagrange Office:

Every Thursday*:   8:30 a.m. - 5:30 p.m.
Closed for Lunch:         12:00 p.m  - 1:30 p.m.


No allery injections are given in the Lagrange Office.

Appointments
Patients are seen in the office by appointment.

To schedule an appointment for our Columbus office, please call us during office hours at 706-324-4012.

To schedule an appointment for our Lagrange office, call 706-885-0070.
When you call, please specify to the receptionist that you are making an appointment for our office in Lagrange.

If you are receiving an injection only, you do not need a specific appointment but should arrive within the posted injection schedule hours.

NOTICE:

Because of the nature of our specialty, we ask that you refrain from wearing perfume or other fragrances to the clinic. These odors may trigger asthma, rhinitis or migraines for some of our patients.

Cancellations
If you cannot keep your scheduled appointment, please notify the office at least 24 hours in advance.


Missed Appointments

Patients who consistently fail to present themselves for scheduled appointments or fail to cancel their appointments 24 hours prior to the scheduled appointment will be considered chronic "no-show" patients and may be denied further access to the clinic.

When new patients who are referred miss their appointment, a letter is sent to the referring physician to let them know of the missed appointment.

Emergencies
If you have an emergency, CALL 911, or go to the emergency room immediately. Do not attempt to contact the on-call physician. The emergency room physician will contact the on-call physician if necessary.

Fees and Payments
Co-payments, co-insurance, and deductibles are due at the time of your visit. Payments can be made in the form of check, cash or credit card.

If other arrangements need to be made depending on your insurance coverage or special circumstances, please see our billing department to make those arrangements

Insurance Policy
Insurance coverage is a contract between the patient and the insurance carrier. In compliance with contracts with some insurance companies, and as a courtesy for our patients, The Allergy Center at Brookstone, P.C., will bill the patient’s insurance for the required payment. The patient is responsible for any co-payment, co-insurance or deductible at the time of the visit. It is the patient’s responsibility to advise us in a timely manner of any change in coverage or eligibility.

All of our providers accept most insurance plans, and are “in network” for all major healthcare plans. We suggest that you check with your insurance company or human resources office if you have questions.

Prior to scheduling your initial new patient appointment, it is our policy to obtain all insurance information to include: name of patient; date of birth; name of employee who carries the insurance, and his or her date of birth and relationship to patient; policy number; name of insurance company, claim address, phone number, and the customer support phone number. We also need the name of your primary care provider and referring provider.

It is our policy to check insurance eligibility on every new patient and to verify coverage benefits prior to the appointment. If we are unable to verify eligibility and benefits due to lack of information or erroneous information, you may be seen as a self-pay patient or the appointment will need to be re-scheduled.

Bring your insurance card for each visit. Again, we are happy to file your insurance claim for you, but ask you to notify us by mail or immediately upon arrival of any name, insurance, home address, or telephone number changes. Changes in insurance coverage should be reported to the clinic staff as soon as possible.
